You've done everything the internet told you to do. New shoes with better arch support. A night splint gathering dust in the closet. Maybe even a round of physical therapy. And yet that deep, nagging ache in the bottom of your foot hasn't budged — or worse, it's gotten sharper the longer you're on your feet at your job in Bethesda or your commute into DC.

Here's what almost nobody tells patients: not every foot pain that feels like plantar fasciitis is plantar fasciitis. One of the most commonly missed look-alikes is fat pad atrophy — a thinning of the natural cushioning under your heel or the ball of your foot. It mimics plantar fasciitis closely enough that patients (and sometimes providers) treat it the same way for months, with little to show for it. The treatments for the two conditions are different, and using the wrong one means paying for relief that never fully arrives.

What Plantar Fasciitis Actually Is

Plantar fasciitis is inflammation and micro-tearing of the plantar fascia — the thick band of connective tissue that runs from your heel to the base of your toes and supports your arch every time you push off the ground. It's the single most common cause of heel pain we see, and it has a signature pattern that's fairly easy to recognize once you know what to look for:

Underneath your heel and the ball of your foot sits a specialized layer of fatty tissue — dense, elastic, and built to absorb an enormous amount of impact with every step. Over time, that cushioning can thin out. Age is the biggest driver (most people lose meaningful fat pad volume by their 50s and 60s), but repeated cortisone injections, high-impact sports, prior foot surgery, and simply years of standing on hard surfaces can accelerate the process.

Once that natural shock absorber wears thin, the bones underneath — your heel bone or your metatarsal heads at the ball of the foot — start taking direct impact with every step. Patients describe it as a deep, bruise-like ache, sometimes with a sensation of "walking on pebbles" or feeling like the padding in their shoes has disappeared. Unlike plantar fasciitis, fat pad atrophy at the ball of the foot is frequently mistaken for metatarsalgia or simply chalked up to "getting older," so it often goes undiagnosed for years.

The Simple Test That Tells Them Apart

Because both conditions cause pain with standing and walking, patients — and even the shoe inserts they buy at the drugstore — often lump them together. Three questions usually separate the two:

Podiatrists also use a quick physical exam called the calf-raise test: rise up onto your toes and hold. If your heel pain sharpens, that increased tension on the fascia points toward plantar fasciitis. If the pain actually eases or barely changes because your weight has shifted off the thinned pad, fat pad atrophy is more likely the source. On an in-office visit, ultrasound or a simple caliper measurement can confirm how much cushioning is actually left — a normal heel fat pad is roughly 1 to 2 centimeters thick, and measurable thinning below that range is a clear diagnostic marker.

How Treatment Differs — and Why the Right Diagnosis Matters

This is the part that actually affects your recovery. Plantar fasciitis and fat pad atrophy are treated with almost entirely different tools, because they're different problems at the tissue level.

For plantar fasciitis, we typically start conservative — targeted stretching, activity modification, and custom orthotics to reduce strain on the fascia — and layer in shockwave therapy or laser therapy for cases that need an extra push toward healing. These treatments work by encouraging the fascia itself to repair.

For fat pad atrophy, stretching and anti-inflammatories don't address the actual problem — there's no inflamed tissue to calm down, only lost cushioning to rebuild. That's why fat pad atrophy responds to an entirely different category of treatment: fat pad restoration injections, such as Liposana and Leneva, which we offer at both our Bethesda and McLean offices. This is exactly why an in-office exam matters more than guessing from symptoms alone — treating only one condition when both are present is a common reason relief stalls.

Is fat pad atrophy permanent once it starts? The natural thinning process doesn't reverse on its own — cushioning that's lost through age or repeated impact doesn't grow back without intervention. That's precisely the gap that fat pad restoration injections like Liposana and Leneva are designed to close, by giving the body a scaffold to rebuild that volume rather than waiting for tissue that won't regenerate on its own.

How long does relief from Liposana or Leneva typically last? Because these treatments work by helping your body regenerate its own tissue rather than injecting a temporary filler, results tend to be considerably longer-lasting than cortisone or cushioning insoles — often measured in years rather than months, though your podiatrist can give you a more specific expectation once they've assessed how much volume you've lost.

Will a cortisone shot fix fat pad atrophy? No — and it can make the underlying problem worse. Cortisone reduces inflammation, but fat pad atrophy isn't primarily an inflammatory problem, and repeated steroid injections are one of the known contributors to fat pad thinning in the first place.
